Depression Checklist
If
you suffer from five or more of the following symptoms of
depression and these symptoms are present for two weeks or
longer without physical cause, you should contact Fairmount Behavioral
Health System 1-800-235-0200. Fairmount provides free
assessments 24 hours a day, seven days a week. A
psychiatrist is on site at all times.
Check symptoms of depression that apply to you:
__ Persistent sad or "empty" mood
__ Loss of interest or pleasure in ordinary activities,
including sex
__ Decreased energy, fatigue, being "slowed down"
__ Sleep disturbances (insomnia, early-morning waking, or
oversleeping)
__ Eating disturbances (loss of appetite and weight, or
weight gain)
__ Difficulty concentrating, remembering, making decisions
__ Feelings of guilt, worthlessness, helplessness
__ Thoughts of death or suicide, suicide attempts
__ Irritability
__ Excessive crying
__ Chronic aches and pains that don't respond to treatment
In the workplace, depression may be recognized by:
__ Decreased productivity
__ Morale problems
__ Lack of cooperation
__ Safety problems, accidents
__ Absenteeism
__ Frequent complaints of being tired all the time
__ Complaints of unexplained aches and pains
__ Alcohol and drug abuse
